New Patient Registration
To register with the practice you must permanently reside within the practice catchment map.

You will need to provide items of photographic identification and also proof of address, complete a registration form and a health check form for each member of the family. This is a mandatory requirement of the practice.
In some cases when you register you may be invited to attend for a health check. This gives us the opportunity to meet you and your family and plan any special health needs you may have and, where there is a family history of chronic disease eg diabetes or heart disease, conduct screening tests
The practice will try to accommodate any specific requests that you have regarding your medical care. If you have any personal requirements or health care needs due to religious, moral or ethical preferences please inform us in writing so that we can update your records accordingly. If you prefer to be seen by a specific practitioner please let the practice know; however this may not be possible in case of emergency.
Please let us know if you change your address or telephone number. Should your move take you outside the practice area, you will need to register with a practice in your new area. Please also ensure that we have your mobile number. This will help us greatly with the text messaging service we are developing.
